how to use the same form for query mode and input mode

This is BHASKARA , I need a help from all , my query is how to use the same form for query mode and input mode , if any body knows about this send me the steps , bcs it is very urgent .. pls guide me

In the form function we have an option .. there we have to choose query only.. for the respective function..

I know this tooo late to answer your question but i jut want to give you the answer.Please open the form Pres F11 and then input the data and then press CTRL+F11.
