regarding the laest DS156 form

Hi,

I have downloaded the form from the internet which showing date as expired on 08/31/2004. So, how to get the new form & where can i get the new form ..Can i fill this form and take it to consulate ?Please let me know .

The DS-156, though its expired on Aug 31, they are still taking it as valid. Before you go to the initial counter, please read the notice board and arrange your documents in the order specified.

So you can fillup the old form. no problems.
