oracle apps new form development

hello friends

this is bhaskara ,
i need a help from all friends , i want to develop a new form for AP module , for extracting the data based on check no. , in this form three fields r there

one is chick id , reset button , & extract buton

this information is extracted from package (ap extraction package )

can u help me the how to devlop the form based on these requirements
bcs i am new in reports pls guid me
