I am creating project "Online Examination" in PHP using mysql.So I enerted quetions in database through input form,in next step I created output form I want to access one question at a time from d/b display on output form. So can you please help me.

yes.its possible to do.first u concentrate on what is the requirement.so u need form for this u refer w3schools.com .this is will be helpful.there are some examples regarding inserting into form and displaying.while displaying u need to be display one question n 4 answer.design the database carefully.
